In addition to the medical care you receive at our office, we provide a variety of administrative services.  These services may not be covered by your insurance.  As such, we collect an optional Administrative Service Fee of $60 annually.  This fee provides unlimited use of these services.

The ASF is not mandatory.  However, if you choose not to pay, you will be charged for all administrative services as needed.  A list of our services and associated fees can be found below.
